How do I replace a lost citizenship certificate?

For lost citizenship certificates the following documents are required for submission: 

  1. A Police report from the nearest Police Station to where the citizenship certificate was lost. 
  2. A letter stating the reason why a replacement citizenship certificate is needed. 
  3. Payment for the citizenship certificate. 
  4. Two (2) Certified Passport size photographs.
  5. A certified valid form of identification.
