The following documents are required to support a lost/stolen passport.
- Completed “Application form”
- Two (2) identical colour photographs one of which should be certified and dated by the same authorized person who signs Section “G” of the application form
- A police report for the lost/stolen passport. If the applicant is submitting an application locally, they may get the police report done (making an appointment) at either of our offices in Jamaica.
- Original Birth Certificate (laminated birth certificate will not be accepted)
- Original Marriage Certificate (if married)
- Adoption Register (where applicable)
- Decree Absolute (for divorcee)
- Deed Poll (if name change)
- Valid form of Identification (eg. current passport, driver’s licence, electoral ID)
- If applicant is 18-years or older without a valid identification, a “Declaration of Identity” is to be completed by their parent/older sibling (also called the Declarant). Declarant must appear in person and provide the following:
- A valid identification of themselves (original)
- An original birth certificate (which must prove their relation to the applicant with mother/father included on birth certificates for both applicant and declarant)
- If applicant is minor, parent/legal guardian should accompany applicant and provide their ID and proof of legal guardianship
- If application is being submitted by someone other than the parent/legal guardian:
- application must be accompanied by a “letter of consent”, signed by parent/legal guardian and notarized by a notary official
- individual submitting the application on behalf of the parent/legal guardian must have a valid identification
