The following documents are required for submission for lost passport:
1. A Police report is required from the nearest Police Station where the passport was lost or stolen
2. A letter stating the reason why a replacement Passport is required.
3. Payment for the replacement passport.
4. Two (2) Certified Passport size photographs
5. A certified valid form of identification
