You may be able to apply for a child’s passport (considering the child is in Jamaica and parents/legal guardians are overseas), by following the steps below:

Requirement for the parent/legal guardian:

  • Parent/legal guardian must provide consent for an adult in Jamaica to apply for the child’s passport. The consent should be in the form of a letter (addressed to PICA). Attached is a sample of our consent letter.
  • Letter must be notarized by an Official who is in the same country as the parent/legal guardian.
  • A copy of the identification for the parent/legal guardian should also be notarized by the same official
  • If consent is being given by a parent, the name of the parent must be on the birth certificate of the child
  • The letter and identification, when completed and notarized, should be emailed to support@picahelpeu.freshdesk.com

Requirement for the Child and accompanying Adult:

  • All minors over 2-years old must appear in person, when submitting an application for passport
  • Minor must be accompanied by the adult to whom consent was given to submit application for the minor; this adult should also take along a valid identification for themselves
  • Sections C and E of the application form should be completed and signed by the adult to whom consent was given to submit the application for the minor.
  • Section G is to be notarised in the name of the adult to whom consent was given to submit the application for the minor. Notary (eg Justice of the Peace) must have known the adult personally for at least 12 months and indicating such on the form.
  • If application is renewal, the expiring/expired passport for the minor should be surrendered at the time of submitting application.
  • If consent is being given by a legal guardian, the official court documents must be submitted to confirm legal guardianship
  • Other documents must include original birth certificate for the child
  • Application cannot be accepted until the consent letter is received by PICA’s email from the parent/legal guardian
